Communication & Facility Requests

Thank you for your interest in submitting a communication and/or facility request!

Please read before completing a form.

• There are two separate request forms.
  • Communication Request Form: bulletin announcements, Mass announcements, etc.
  • Facility Request Form: special meetings, events, fundraiser, etc.
• Events or facility usage at St. Mark must be approved before submitting a Communication Request Form.
• Once a facility request has been approved, you may complete the Communication Request Form for marketing and promotions if needed.
• The purpose of the Communication Request Form is for marketing and promotions only.
• For both forms, users will receive an automated copy of the form in their email upon submission.
